How to Create a New Form

Creating a new form in the Prestartr app is a straightforward process that can be started from several different locations. This guide will walk you through the steps to successfully create and configure a new form.
Starting Points for Form Creation
You can begin the process of creating a new form from multiple modules within the app:
- Dashboard (if configured for quick access)
- Forms
- Jobs
- Plant
- Work Orders
- Companies
- People (for Users only)
The "Add New Form" Menu

Regardless of where you start, you will be presented with the Add New Form menu. This menu has four key fields that link your new form to the correct record in the app.
Template: This is the type of form you want to create.
Example: 'Take 5', 'Site Inspection', 'Training Record'.
Entity: This is the category that the form relates to. This field often auto-populates based on the Template you select.
Example: A 'Vehicle Inspection' template will set the Entity to 'Plant'.
Record: This is the specific item you are creating the form for. You will select this from a list.
Example: If the Entity is 'Plant', the Record would be where you select the specific vehicle, like 'Excavator-001'.
Zone: This field allows you to assign the form to a specific area or location. The options shown will be filtered based on the selected Entity and Record.
Create New Form

Once all fields are correctly filled, tap the Create New Form button to generate the form.
Important Note: All newly created forms are automatically saved with a Draft status.

Prestartr Training Records
- Log in to the Prestartr app and navigate to the Form module.
- Tap the Add icon in the bottom right or the Add New icon in the upper right.
- Select the Training Record form template.
- Fill out the required details of the Training Record such as Certificate/License/Course Name, Certificate/License/Course Number, Course Provider, and Assessment and Expiry dates.
- Upload any relevant Files or Photos.
- Tap Save and check your Training Record draft is correct. You can Edit, Submit, or Delete the form.
- Once submitted, you can still Delete the form or go back home to the Dashboard.

The Take 5 form in Prestartr is designed to help you identify hazards and manage risks before starting a task. Follow these steps to create, document, and submit your assessment.
- Initiate the Form
- Select Project Details
- Enter Required Information
- Add a Risk Assessment (Optional)
- Sign and Save as Draft
- Submit the Form

Ensure the assessment is logged against the correct location:
- Select the appropriate Job from the dropdown menu.
- Select the specific Zone if required for your site.
Enter Required Information
Fill out all fields marked with a red asterisk (*). These are compulsory and must be completed to submit the form. Key fields usually include:
- Company and Division
- Respondent name
- All Yes/No safety checkboxes
Add a Risk Assessment (Optional)
If a specific task requires a deeper dive into hazard management, you can attach a detailed risk assessment:
Tap the Add Job/Task button.
Enter a Description of the specific job or task.
Select the relevant Hazard and the Initial Risk Rating.
Input your Control Method (how you will make it safe) and the Residual Risk Rating.
Add any additional comments and tap Save to return to the main Take 5 form.
Sign and Save as Draft
Before finalizing, you must authorize the document:
Provide your Signature in the designated field.
Tap the Save button.
Note: Saving at this stage places the form in Draft status. It is saved to your device but has not yet been formally submitted to the system.
Submit the Form
Review your entries for accuracy. When you are ready to finalize the assessment, tap Submit. Once submitted, the Take 5 is confirmed and logged in the system.

How to Complete a Plant Prestart
Follow these steps to complete your daily equipment inspection.
Start a New Prestart
There are two ways to begin:
- From the Dashboard: Tap Complete Plant Prestart.
- From the Forms Module: Tap Add New.
Identify the Equipment
- Select the specific equipment from the list or scan the QR code located on the machine.
- Enter the current odometer reading and select the job site.
- Tap Complete Daily Prestart - Generic Plant.
Conduct the Inspection
Review and respond to all questions under the PRE-OP FLUIDS and EXTERNAL INSPECTIONS sections.
- Dynamic Fields: Some answers will trigger additional fields. If prompted, provide a comment and upload a photo to document any issues.
- Sign-off: Once all questions are answered, select the name in the Checked By field and provide your digital signature.
Save, Edit, or Submit
- Save as Draft: Tap Save. This places the form in Draft status, allowing you to edit or update details later.
- Submit: Tap Submit to finalize the prestart. Note that once submitted, the form can no longer be edited.
- Delete: If you started the form in error, tap the Delete button to remove it.

Check-in and Check-out via QR Code
Workers can check in and check out by scanning the QR Code on the Daily Site Prestart.
From the dashboard, tap the option for Scan QR, this will open the camera on your device.
Scan the QR code found on the Daily Site Prestart to Check-in.
When ready to check-out, repeat the process to scan the QR Code again.
Checking in or out will display on Submitted prestarts in the Attendance Register.

Start a New Incident Report

- From the Dashboard: Tap Incident Report.
- From the Forms Module: Tap Add New.
Update the Report questions
Review and respond to all questions on the Report.
- Add relevant Files
- Add Involved Persons and Witnesses
Save, Edit, or Submit
- Save as Draft: Tap Save. This places the form in Draft status, allowing you to edit or update details later.
- Submit: Tap Submit to finalize the report. Note that once submitted, the form can no longer be edited.
- Delete: If you started the form in error, tap the Delete button to remove it.
Start a New Incident Report
There are two ways to begin:
- From the Dashboard: Tap Incident Report.
- From the Forms Module: Tap Add New.
Update the Report questions
Review and respond to all questions on the Report.
- Add relevant Files
- Add Involved Persons and Witnesses

Tap on the button for either Persons Involved or Witnesses, as required.
Complete the following fields on either subform:
- First Name - Surname - Mobile Number
- Signature - Date
Tap Save to add the subform to the Incident Report.
Save, Edit, or Submit
- Save as Draft: Tap Save. This places the form in Draft status, you can edit details later.
- Submit: Tap Submit to finalize the report. Once submitted, the forms cannot be edited.
- Delete: If you started the form in error, tap the Delete button to remove it.
