How to Complete an Incident Report

Follow these steps to complete Incident Form.

Start a New Incident Report

There are two ways to begin:

  •   From the Dashboard: Tap Incident Report.
  •   From the Forms Module: Tap Add New.

Update the Report questions

Review and respond to all questions on the Report.

  •   Add relevant Files
  •   Add Involved Persons and Witnesses

Save, Edit, or Submit

  •   Save as Draft: Tap Save. This places the form in Draft status, allowing you to edit or update details later.
  •   Submit: Tap Submit to finalize the report. Note that once submitted, the form can no longer be edited.
  •   Delete: If you started the form in error, tap the Delete button to remove it.
