Creating a Defect in Prestartr
Defects can be created in two ways:
- Manually via the Defect Dashboard
- Automatically through custom form fields with a Defect Configuration.
Click the Dashboard tab for Manual Instructions.
Click the Form tab for Automatic Form instruction.
Dashboard
To create a new Defect from the Dashboard, click New Defect.


When creating a new Defect, you record the relevant information from the header, the Linked Entities, Details, and Files as needed to document the Defect correctly. There are no required fields when creating a Defect.
The following fields require setup in other areas prior to use when creating a Defect:
- Defect Type – configured in the Catalogue
- Zone – set up via the People or Plant modules
- Defect Status – configured in Defect Options
Click Create Defect when finished to add the Defect to the Dashboard.
Form
Supported Field Types for Defect Configuration
The following Field Types on a Form Template will support the creation of a Defect via a Defect Configuration:
- All Question Fields
- Combo Box Field
- Multiselect Combo Box Field
To create a Defect as a result of a form response, you would need to set up a Defect Configuration on a Form Template field, as well as a Show/Hide rule.

Set up a Defect Configuration

On a Question, Combo Box, or Multiselect Combo Box field, click Edit Defect Configuration to open the configuration window.

From this window you can set the Priority, Type, as well as link specific form fields for Photos and Comments.
- The Photos drop-down displays a list of Photo form fields on this template.
- The Comments drop-down displays Text or Long Text form fields on this template.
Defect Priorities is a fixed list:
- Very Low
- Low
- Medium
- High
- Very High
Defect Types are set in the Catalogue - see Catalogue Basics for details on creating additional Defect Types.
When adding a Defect Configuration, the only required selection is the Priority.
Click the Save button to commit your changes to the Defect Configuration.
After creating your Defect Configuration, the next step is to setup a Show/Hide rule.

Click Edit Show/Hide Rules
Click Add New Rule.

From the Show/Hide Rules menu, select the If Value for the value that will trigger the Defect creation - such as Fail or No.
Check the Create box for Defect.
Click Save to close the Show/Hide Rules menu.
Click the Save icon on the form field.
Click Update to save your Form Template.
When this Form Template is used from this point onwards, the triggering field will create a Defect when the field is completed and the form is marked as Submitted.

A new tab will be added to the submitted form - Defects.
From here you can see the details of when the defect was created, the status, as well as which field from the template created the Defect.
